About the Role:

The Nurse Practitioner (NP) works within an interdisciplinary team to provide leadership, clinical care, and system navigation as part of the Outreach Team in Brockville and the surrounding region. This role is designed to meet the health needs of vulnerable and underserved populations through a mobile, community-based model of care. This role is focused on delivering trauma-informed, harm-reduction-oriented care in shelters, addiction recovery centers, transitional housing, encampments, and socially vulnerable communities.

POSITION TITLE:     Nurse Practitioner - Outreach

DEPARTMENT:         Primary Care

REPORTS TO:          Director, Integrated Care

REPRESENTATIVE ACTIVITIES:

  • Provide leadership and guidance to the Outreach Team in Brockville
  • Provide mobile, client-centered primary care including assessment, diagnosis, treatment, health education, chronic disease prevention and management, and counselling.
  • Work collaboratively with community partners, local organizations, and housing/shelter programs to support wraparound services.
  • Deliver care using a trauma-informed, harm-reduction lens to individuals experiencing homelessness, substance use disorders, and/or mental health challenges.
  • Serve as a liaison and lead for the Outreach Team, coordinating activities, mentoring staff, and ensuring effective team-based care.
  • Participate in case conferences, interdisciplinary rounds, and collaborative planning with external service providers.
  • Keep complete, accurate, and timely records of client visits using the electronic medical records system (EMR).
  • Provide care in non-traditional settings, including temporary shelters, outdoor encampments, and transitional housing, using mobile tools and supplies.

  • Incorporate a determinants of health framework and health promotion into all aspects of care.
  • Promote and model interdisciplinary teamwork and mutual respect.
  • Maintain licensure, continuing education, and professional development.
  • Contribute to staff mentorship, orientation, and team development.
  • Support RCHS health promotion activities and strategic initiatives.
  • Adhere to Occupational Health and Safety policies and procedures.

QUALIFICATIONS:

  • Master of Nursing degree with successful completion of a post-graduate Nurse Practitioner program.
  • Registration in the Extended Class and in good standing with the College of Nurses of Ontario.
  • Current membership with the Nurse Practitioners' Association of Ontario (NPAO), including professional liability insurance.
  • Minimum of 3 years' experience working with marginalized, under-housed, or socially vulnerable populations.
  • Demonstrated clinical experience and comfort in outreach, mobile, or non-traditional practice environments.
  • Strong health assessment, decision-making, and clinical documentation skills.
  • Commitment to harm reduction, trauma-informed care, and anti-oppressive practice.
  • Experience supervising or mentoring clinical and non-clinical staff an asset.
  • Comfort with EMR systems and mobile documentation tools.

  • Ability to work flexible hours and travel within the region as required.
  • Valid driver’s license and access to a reliable vehicle.
  • Satisfactory Canadian criminal record check and vulnerable sector screening.
