Assistance in Applying to the Canadian Dental Care Plan

What:

Rideau Community Health Services, in partnership with The Falls Food Hub will be offering assistance with applying to the Canadian Dental Care Plan

To qualify, you must meet ALL 4 requirements:

·        No access to private dental insurance

·        Family net income under $90,000

·        Canadian resident for tax purposes

·        Filed a tax return for the previous year

What you need to apply:

• Social Insurance Number (SIN) for each applicant, and your spouse or common-law partner (if applicable)

• Date of birth

• Full name, and mailing address

• List of dental coverage through government social programs (if applicable) 

Applications for individuals aged:

• 55-64 are opening May 1

• 18-34 are opening May 15

• 35-54 are opening May 29

When:

·        Monday May 5 and Monday June 2

Where:

·        The Falls Food Hub Community Lunch

·        30 McGill St N, Smiths Falls

Time:

·        11am-1pm
